athletic activities is a privilege that may be withdrawn for violations of
the Athletic Code of Conduct and Expectations, hereinafter Code of
Conduct.
- I have read the Code of Conduct and thoroughly understand the
consequences that I will face if I do not honor my commitment to the Code
of Conduct.
- I understand and realize that there is risk of
injury in participating in athletic activities.
- I have read the Athletic Handbook and agree to
its terms.
- I understand this is binding while a
student/parishioner of St. Mary Catholic Church.

- I have read the Code of Conduct and understand the
responsibilities of my son or daughter as a participant in athletic
activities in the St. Mary Catholic Church..
- I pledge to promote healthy lifestyles for all
student athletes of the St. Mary Catholic Church.
- I understand and realize that there is an
assumed risk of injury involved for my son or daughter as a participant in
athletic activities.
- I have read the Athletic Handbook and agree to
its terms.
- I understand this is binding while my child is a
student/parishioner of St. Mary Catholic Church.

on file in the Athletic Office before a student may participate in athletic
events for St. Mary Catholic Church.

CONSENT FORM
- I hereby pledge to provide
positive support, care, and encouragement for my child participating in
youth sports by following this code of ethics:
- I will encourage good
sportsmanship by demonstrating positive support for all players, coaches
and officials at every game, practice or other youth sports event.
- I will place the emotional and
physical well being my child ahead of any personal desire to win.
- I will insist that my child
play in a safe and healthy environment.
- I will provide support for
coaches and officials working with my child to provide a positive and
enjoyable experience for all.
- I will demand a drug, alcohol,
and tobacco free sport environment for my child and agree to assist by
refraining from their use at all youth sports events.
- I will remember that the game
is for children and not for adults.
- I will do my best to make youth
sports fun for my child.
- I will ask my child to treat
other players, coaches, fans, and officials with respect regardless of
race, sex, creed, or ability.
- I will promise to help my child
to enjoy the youth sports experience within my personal constraints by assisting
with coaching, being a respectful fan, providing transportation, or
whatever I am capable of doing.
- I will require that my childžs
coach be trained in the responsibilities of being a youth sports coach
